2026 CBCC Request for Proposal

We are pleased to announce the launch of the GHN Community-Based Care Coordination (CBCC) Request for Proposals (RFP) for 2026.  

This year, there are three contract tiers available to apply for, as well as an Infrastructure Fund Application, check all that apply in the application form provided. Please refer to the CBCC RFP Guidelines document for detailed descriptions of each tier and the required supporting documentation.​

Note: If your organization is only interested in the Tier 3 Contract (Access to Workforce Development Training), you do not need to complete the RFP application packetFor Tier 3 only inquiries, please indicate your interest by sending an email to socialcarenetwork@greaterhealthnow.org, and a GHN team member will connect with you for the next steps.  

For Tier 1, Tier 2, and Infrastructure Fund Applications, please follow the RFP 2026 Guidelines document.  

Required Submission Materials:  

  • Narrative Proposal (may be completed on your organization’s letterhead)  
  • Completed Request for Proposal Application Questionnaire (provided)  
  • Completed Community-Based Care Coordination Budget (template provided)  
  • Completed 2026 Infrastructure Fund Application (if applying for additional funding)  
  • Infrastructure Budget Template (template provided)

Outlined timeline and Key Dates   

October 2025 – RFP information posted to GHN website  

  • October 20–31 – Open office hours for CBO RFP support   
    • Every Tuesday from 11:00 AM to 1:00 PM via Microsoft Teams  
  • November 7, 2025 – Proposals due 5:00 PM PST  
  • October–November 2025 – Proposals reviewed as received  
  • December 2025 – Announcement of successful applicants  
  • January 1, 2026 – Contract start date 

Open Office Hours:  

We will be hosting open office hours every Tuesday from 11:00 AM to 1:00 PM via Microsoft Teams to answer questions about the RFP process, application tiers, and submission requirements. Teams
