Emergency Medical Services Innovative (EMSI) Update - Greater Health Now

Emergency Medical Services Innovative (EMSI) Update

Greater Health Now has been working with several fire departments in the Greater Columbia region. The goal of the fire departments that are participating in the EMSI program are:

1. Strengthening the primary health care delivery system

2. Implement change in patient outcomes and reduces health care cost by

    a. Reducing non-emergent 911 calls

    b. Reducing non-emergent ED visits

    c. Reducing inpatient hospital readmissions

3. Meeting unmet health care needs

How are the fire departments going to be able to meet the goals above, you might ask yourself? The answer is by being innovative. Fire departments have the flexibility to choose what conditions and/or risk states they will be focusing on. Diabetes, congestive heart failure, falls, chronic obstructive pulmonary disease, high utilizers of the 911 system and high ED utilizers are some of the conditions and/or risk states that fire departments have chosen to focus on. Why are they focusing on these conditions and/or risk states? The fire departments are focusing on these conditions and/or risk states because they know their community and in doing so they understand the community members that have these conditions. As a result the fire department can make a meaningful impact on those individuals. Some of the fire departments are already collaborating with local hospitals to identify patients that can benefit from the EMSI program. In the short time that the EMSI program has been going, there have already been wins by some of the fire departments by identifying and contacting community members that are either high utilizers of the 911 system or ED or at risk of becoming a high utilizer connecting them with resources.
